Aledade
Dunkirk Family Practice also participates with Aledade, which is an Accountable Care Organization (ACO) specifically for our Medicare patients. This program allows us to work proactively with our patients to help them get and stay healthy, not just treat them when they are sick.
Maryland Primary Care Program (MDPCP)
Dunkirk Family Practice is participating in MDPCP. The MDPCP supports the overall health care transformation process and allows primary care providers to play an increased role in prevention, management of chronic disease, and preventing unnecessary hospital utilization.
Telehealth
We are now offering telehealth to our sick patients until further notice. These office visits include, minor sick visits, and follow-up appointments to our existing patients.
